What BBL is and Isn’t
Regardless of its very misleading “L”, a BBL is NOT a butt lift. It is a butt augmentation with your own fat. Brazilian Butt Lift (BBL),or gluteal fat grafting, helps women achieve the desired hourglass shape which was traditionally only achievable via restrictive undergarments and corsetry.

A large weight loss may result in deflation, significant excess skin above the buttocks and upper thighs with a sagging of the buttock and thighs.

WHAT IS PRECISION BBL™ ?

What Makes Buttocks TRULY Beautiful and Sexy?

Multiple studies have repeatedly shown that all humans, irrespective of their age and ethnicity, perceive certain body proportions as attractive. In the face, it is the “Divine Proportion” of 1.618 while with the female figure it is a WHR (waist-to-hip ratio) of 0.7. According to evolutionary psychologists, beauty was never really left to the variability of the “eyes of the beholder”.

We all know what we like and do not like in female buttocks. But, while ethnic considerations may diverge slightly, our tastes are universally united by several consistent features that make buttocks attractive to all of us. These are, beginning high and moving lower: 1. Increased forward curvature of the lower back - Normally, the low back has a curvature (lordosis). In sexier buttocks, women demonstrate a more pronounced lumbar lordosis (hyperlordosis) just above the buttocks. In these women, instead of their lower back spines joining the buttocks in a straight, vertical fashion, the low lumbar area of the spine have an accentuated forward curvature which elevates the sacrum making it more horizontal by 5-7 degrees. This results in an impression of both elevated and larger buttocks. 2. Clear separation of the buttocks both superiorly and inferiorly. The buttocks should appear to stand alone rather than merging indistinctly into the lower back and thighs. 3. Sacral (Venus or Butt) Dimples. The supragluteal fossae, the paired lateral lumbar indentations forming the highest side points of the sacral, V-shaped back triangle are seen in most women. These indentations are located superficial to the posterior superior iliac spine and the sacroiliac joints (the junction where the iliac bones of the pelvis join the sacrum).

| 4. Sacral Triangle (in red on Left) - This inter-cheek V-shaped depression (Sacral Triangle) provides a well-defined origin and contrast for the curves of the adjacent buttock hemispheres. The triangle boundary lines are formed by the well-defined insertions of the gluteus maximus muscles into the lumbodorsal aponeurosis.
5. The sacral Triangle is the lower half of the Rhombus (Diamond) of Michaelis (in Yellow on Left). This kite, diamond or rhombus-shaped concave area, which was portrayed by artists for centuries, was formally described by the 19th Century German Gynecologist, Gustav Adolf Michaelis (1798-1848) as an indicator of pelvis width. The area of the rhombus is delineated by 4 summits; At the top, the third or fourth lumbar vertebra, along its sides, the Sacral Dimples and inferiorly, the junction of the sacrum and tail bone as well as the beginning of the vertical intergluteal buttock crease. A well-defined Rhombus of Michaelis separates the back and buttocks and thereby imparts definition and beauty to the female buttocks. In the recent decade, in a subconscious effort to create more definition and emphasize the rhombus, we have seen a surge of lower back tattoos placed on this area.

5. Defined, Rounded, FULL buttocks – The most attractive buttocks are well-defined hemispheres or spherical caps, whose maximal projection is centered about the upper or middle segments of the buttock. The buttocks are formed by the gluteal muscles (gluteus maximus and medius) superimposed by TWO distinct layers of fat. The volume of the buttocks can be increased by enlarging the gluteal muscles with serious and long exercise, by placing gluteal implants or by SAFELY transferring your own fat ABOVE the muscle (see Precision BBL™ below) .
Please, note on the right how the forward sweep (hyperlordosis) of the lower back accentuating the buttocks, the well-defined sacral triangle and the roundness of the buttock spherical caps – all combine to create a sexy butt. 6. Potential Complications of BBL (Complications refer to possible, unexpected and undesired events that may occur even with acceptable surgical technique)
1- Significant Fat loss (necrosis) with loss of butt volume due to lack of blood supply to the grafted fat 2 – Buttock surface lumpiness, irregularity and occasionally oil cysts. 3 - Lack of symmetry 4 – Infection 5 – other potential complications of Surgery and, rarely 6 – Death from fat going through the gluteus maximus muscle into its underlying blood vessels and then reaching the lungs (Pulmonary Fat Embolus, PFE) causing shock and death. (The ONLY thing worse than BBL’s in which fat is placed in the butt muscle, are so called “nonsurgical butt enhancement” and black market “Butt Shots” where non-FDA authorized substances (Silicone caulking, Hydrogel or worse, Fix-A-Flat, or cooking oil) are injected into the buttocks. The long-term results are hideous and impossible to reverse. Worse yet, large blood vessels travel through our gluteus muscles. If anything is injected into these vessels, those substances can quickly travel to the heart and lungs, resulting in death. No reputable Plastic surgeon would ever offer this “treatment”).

10 Things you MUST know to Stay SAFE

1. Choose the right Plastic Surgeon. Picking the wrong BBL surgeon, can be costly, painful and even deadly. No two doctors are the same. Training and Experience are very important (It does not help you if your chosen surgeon brags of having done thousands of BBLs but has done them carelessly, rapidly and poorly).
Check your surgeon's credentials (their college and medical school education, surgical training, certifications, society memberships, hospital staff membership, reviews etc). And when you are done, check him/her again.
A few pointers; Just because someone claims they are “Board Certified” does not mean they are certified by The American Board of Plastic Surgery (of The American Boards of Medical Specialists (ABMS)) Check their certification HERE (https://www.certificationmatters.org/find-my-doctor/ ).
No hospital or multispecialty outpatient surgery center, where regulations are stricter and membership depends on proper ABMS board certification, would allow untrained/uncertified surgeons on their staffs performing outside their field of training (ie “Scope of Practice”).
But in Florida, Tennessee and most states in the USA, anyone with a MD degree regardless of their training, can practice ANY branch of Medicine/Surgery in their clinic as long as they get consent / permission from the (uninformed) patient. (More dangerous, we have seen instances when Nurses, with minimal training, have performed “BBLs” in their offices).

A slick presence on Instagram, Tik Tok, Instagram or Facebook is rarely a guarantee of a great and safe Plastic surgeon and it never ensures professionalism, dedication or clinical Excellence. It is the result of repeated and expensive marketing and an extraordinary amount of time spent on taking videos than on your care and privacy.
Assume the Before/After glamour photos you are seeing have been heavily photoshopped (much like the photos of burgers/fries in fast food advertising) and in the era of AI consider all slick ads to likely be fake/forgeries.
Advertising is expensive and commonly preys on working class minorities, At a single well-known Miami BBL clinic, eight women died over the course of six years; Seven of these victims (88%) were working-class Black and Latina women. (The clinic has repeatedly changed its name to avoid being recognized on Google searches)
Please, do not pick a surgeon who dances, prances or swaggers in the operating room during surgery on Social Media. Your surgeon, like a pilot in flight, should act professionally and be totally focused on you. Dancers and those hamming it for their social media crowds care much more about pushing the limits and standing out than for your safety. You are just a prop for them. (Imagine how you would feel if at 35,000 feet your pilot came out of the cockpit and began twerking)
2. Conveyor Belt BBL (high output, rushed, impersonal, low quality) Clinics especially in Florida and abroad) - Everything has a cost - THERE ARE NO DEALS and no free lunch
Most surgical disaster cases and BBL deaths have, for the most part, occurred because of improper and rushed surgical technique in high-volume, discount chop shops “clinics” / “Institutes” / “Centers” in South Florida and abroad which crank out multiple low quality BBLs all day long. Their prices are seductively low, the turnover is very high and the doctors (mostly non-Plastic surgeons) are contracted employees who perform as many as 8 to 11 BBLs a day and are underpaid for each BBL by the clinic owner.
How then could these clinic physicians perform 8 to 11 cases a day? (Let’s do the math. Assume they start at 7AM and stop working at 9PM. That means 14 hours of surgery or 90 minutes per BBL! Do you really think the quality of a 90 min or less BBL comes close to a 3 hour BBL?).
Do you really think the surgeon even knows the name of the fifth or eighth woman he/she is operating on? In two recent Florida BBL deaths, two surgeons who were operating late into the night, could not even recall their dead patients’ names when EMT/Police arrived. Is this the kind of attention you want?
For your safety, you want your airline pilot and surgeon to be fresh, focused and rested. You do not want a tired, disinterested and distracted surgeon rushing through your BBL. Do you really think that a surgeon is as rested after 9 hours of surgery?
It is not an accident that fatal pulmonary fat emboli (PFE), infections and butt irregularities happens most frequently in these high turnover, production line/conveyor belt BBL clinics which spend large sums on internet marketing, less on patient safety and post-operative care and employ many minimally trained non-plastic surgeons.
3. Make sure you MEETyour surgeon BEFORE you pay for your operation to see if you like him. It may be the most important meeting of your life. Never agree to meet the surgeon on the day of your surgery. (Would you agree to meet your husband for the first time at the altar?)
4. Make sure your surgeon uses experienced certified anesthesiologists and anesthetists. There are no deals in Plastic Surgery. You essentially get what you paid for.
5. Make sure your BBL is done in a AAAASF or Medicare certified facility. These facilities must adhere to a much higher standard in sterility, nursing, power backup and reporting to state and federal authorities. All doctors and certified facilities worked hard to obtain and maintain such facility certifications and are proud of it. Ask to see their certification. If they refuse to show it to you, please, leave.
6. Using surgically sterile instruments and unreused fat collection canisters. It is well known that to keep costs low, and do more procedures, some discount surgery center bend the rules on sterility and are rapidly and incompletely sterilizing their instruments and may be reusing some single use tubing. (https://www.youtube.com/watch?v=iUagzS8n6u8 ) 7. Never inject the fat under pressure as the cannula moves forward. To reduce the pressure of fat injection, always inject when it is stationary or being pulled back. 8. Fat losses (necrosis) by over-inflation with fat. Just like you cannot make a size 4 dress stretch to effortlessly and comfortably fit a size 14 woman, surgeons are limited by the existing inherent capacity and volume of the buttock deep fat compartment.
The tightness/looseness/compliance of the buttock skin, and especially of the underlying Superficial Gluteal Fascia buttock layer (see below), will determine just how much fat can be placed under them. A tight fascial envelope will allow less fat grafting but could be associated a greater upward rotation (“lifting”) of the flat buttock.
The grafted fat is a LIVING filler which requires the adequate supply of oxygen and nutrients (ie circulation) to survive. Pressure (as with sitting) on the grafted butt areas will decrease the blood flow. Moreover, the more fat is forced in the, the higher the pressure created in that space, causing reduced blood flow there and the higher the proportion of lost fat (fat necrosis).
Loss of fat varies from person to person and even from butt cheek to butt cheek in the same woman. To reduce the proportion of fat graft losses, experienced BBL surgeons limit the amount of fat injected to allow the blood vessels in the deep buttock fat layer to reach and nourish the grafted fat. Much bigger is rarely better. The less pressure (sitting) you put on the grafted cheek, the more blood flow it will have and more would survive.
9. Aftercare is crucial. Understand and strictly follow your surgeon’s post-operative instructions. Offloading (reducing sitting and pressure on the grafted buttocks), PROPER compression garments, lymphatic massages and avoiding salty foods and food preservatives to lower swelling. If your surgeon tells you can safely fly/drive home a few days after a BBL and follow up in an ER or Family Practice clinic with another physician consider it a major red flag. You are being dumped. An ethical surgeon treats his patients himself and does not tell them to go “elsewhere” after the surgery.

BUT THE most important rule is
10. Avoid fat injections into the buttock muscles When modern BBL began in the 1990’s, surgeons placed fat in all layers of the buttocks, including the muscle. The thinking at the time was that muscles have a great blood supply which would enable a greater percentage of the transferred fat to survive. That is true.
What they did not understand in those days was that the gluteus muscles varies in thickness and has a variable supply of large veins both within and underneath it. When one of these large veins is accidentally injected with fat, that fat can travel to the heart and then lungs where it can cause a deadly pulmonary blockage (embolism).
Surgeons who reported injecting fat into the butt muscle had a 403% increase in fatal and nonfatal lung (pulmonary) fat clots (PFEs).

Once Plastic surgeons came to understand this situation, all major Plastic Surgery societies, both in the US and abroad, have strongly recommend against placing fat in the gluteus muscles. As a result, the BBL date rate fell from its high of 1 in 2,351 BBL cases several years ago to 1 in 14,952 BBL procedures in 2020 (which states that a BBL which avoids the muscle is now safer than tummy tucks, which have a mortality rate of 1 in 13,147). (Please note than this lower BBL death rate only applies to only BBLs performed by ABPS board-certified plastic surgeons. These stats do not apply to BBLs performed by “cosmetic surgeons” and other types of providers who are performing BBLs regardless of their training and certification despite not being board-certified in plastic surgery.)
But the persistent problem is that that a BBL, performed without Ultrasound guidance, REMAINS a “blind operation”. Doctors do not have X-Ray vision and even experienced ABPS-certified plastic surgeons cannot be sure by watching and feeling if they are injecting the fat in or above the muscle.
So how can Plastic surgeons make sure they are NOT injecting fat in the muscle?
Simple. Ethical Plastic surgeons who want to carry out safe and predictable BBLs must be able to SEE both the gluteus muscle and the exact location of the tip of their fat transferring tube/cannula and make sure that it is not in the muscle.
How is that possible? Precision BBLTM Ultrasound-Guided Fat Grafting of the Buttocks
Since July 2023, Ultrasound-Guided BBL fat grafting is the LAW in Florida for all BBLs (HB 1471) but it is not yet mandated for BBLs done in Tennessee or other states.

Understanding Buttock Anatomy is Crucial for a Safe and Successful BBL
Our skin, fat and muscles are organized in discrete layers with the fat compartment under the skin being divided by a thin layer called Scarpa’s fascia (in the buttock, it is called the Superficial Gluteal fascia) into superficial and deep fat compartments. The Deep Gluteal fascia covers the outer surface of the buttock muscle, the Gluteus Maximus.

Consultation - During your Brazilian Butt Lift plastic surgery consultation, Dr. Aldea will discuss your goals for treatment and review your medical history to determine if there are any factors that would preclude you from undergoing the butt lift surgery. The best candidates for Brazilian Butt Lift are men or women with realistic expectations who are in relatively good health who are not too thin and wish their lower backs and buttocks improved. If buttocks are to be augmented with a fat transfer, you must have a ready source of fat elsewhere in the body we can borrow from. If you are too thin and wish to have the Brazilian Butt Lift, you may need to gain 10-15 pounds to produce enough fat for transfer before this operation is seriously considered. The BBL procedure starts with meticulous pre-operative planning. The current appearance of the buttocks and lower back are analyzed and areas of needed improvement are noted and discussed with you. The volumes of fat required for transfer and grafting are estimated. Then, the rest of the body is analyzed to see which areas of the body (tummy, flanks, outer thighs etc) could be liposuctioned to yield this fat. Although each case is different, at least 400cc of fat are needed for each buttock. This translates to your having to have and for us being able to remove at least 1,000 cc (or 2.2 pounds of fat) for buttock grafting. For your comfort and safety, most Plastic surgeons prefer to do this operation under general anesthesia, so you'll sleep comfortably through the operation. In surgery, the fat to be grafted is vacuumed gently to reduce damage to its cells. The fat is then gently purified to remove any debris and fat cells which were damaged. Only the best donor fat cells are then placed in the buttock under direct ultrasound guidance (Precision BBL™). Using a few tiny needle holes, the fat is then injected into the buttocks in multiple small tunnels.

at various depths so that each fat cell will have a better blood flow to it and increase its survival and persistence. As the fat is grafted, the buttocks are carefully observed and checked throughout both for symmetry and correction of goals. The Brazilian Butt Lift Butt Augmentation and Lift is slow and painstaking procedure which is focused on proper retrieval and transfer of fat grafts. Because it lasts 4-8 hours many surgeons prefer not to do it. It would be fair to say if you hear of somebody doing a fat grafting Butt Augmentation-Brazilian Butt Lift in less than four hours chances are the grafts may not last.

Recovery - If you live out of town, we would ask that you stay in the Memphis area for 10-14 days after your surgery during which time we could follow your progress.
There will be some swelling for 6-8 weeks, but you should be able to resume your normal activities 4 weeks after the procedure. For 4-6 weeks after surgery you will need to wear a surgical compression garment which will help contour your figure, stabilize the fat grafts and reduce swelling, thereby, improving tissue circulation and nutrition. The fat grafts should be regarded as freshly planted seedlings. You must allow them to establish a stable blood supply. Any undue pressure and shearing forces on the fat grafts may disrupt this important process. Therefore, you will need to sleep on your stomach and except for bathroom activities, you will need to avoid sitting on the grafted buttock areas for 2 weeks after surgery. After that until 6 weeks after the procedure, you may slowly resume sitting for necessary activities, but use a pillow and keep the sitting to a minimum.
On the average, depending on the nature of your occupation, you may be back to work: average 2 - 4 weeks. More strenuous activity (including gym): average 4 - 6 weeks or more.
